Effect of Medial Branch Block in Chronic Lumbar Facet Joint Pain. Retrospective Study -Two Years Follow Up-

Abstract
Objective: This study was to evaluate the outcomes of me-dial branch block (MBB) in chronic lumbar facet joint pain and the relationship between their impact on treatment out-come and other factor, such as the radiologic finding. Method: From June 2008 to June 2012, 412 patients re-ceived MBB for chronic lumbar facet joint pain. A total of 126 patients fulfilled inclusion criteria. The clinical outcomes were measured by Verbal numeric scale (VNS) and Oswestry disability index (ODI) before treatment, 3 months, 6 months, 12 months 18 month and 24 months after MBB. Radiographic analysis included measurement of vertebral lordotic angle. After 24 months, patients¡¯ satisfaction was classified to five categories: excellent, good, fair, poor or fail. Statistical analysis of both radiographic and clinical pa-rameters along with treatment outcome was performed to determine any significant correlations between the two. Results: VNS and ODI was improved 3 month after the injection and continued to maintain until 24 months. Significant improvement were observed with pain relief (£¾50%), functional improvement (£¾40%) and the patients rat-ed their satisfaction level as "excellent" or "good" after 24 months in 72.2%. The radiographic and clinical parameters were not significantly correlated with treatment outcome. Conclusion: Our retrospective study demonstrated that the medial branch block provided significant pain relief and functional recovery to the patients with chronic lumbar facet joint pain. A placebo-controlled prospective randomized double-blind study should be conducted in the future to evaluate the treatment effects.
